Kidneys are the most commonly involved organs in extranodal sites of lymphoma, being found in 30% of patients with non-Hodgkin lymphoma [1]. Nevertheless, primary renal lymphoma [PRL] is a rare neoplasia, corresponding to less than 1% of all extranodal lymphomas [2]. We report a primary renal lymphoma in a 70-year-old woman presenting with chronic lower pain due to a solid mass in her left kidney treated with radical nephrectomy. The evaluation of the biopsy specimen revealed a marginal zone lymphoma that was treated with chemotherapy. This rare condition is a differential diagnosis with other renal masses and can be a clinical image challenge, making the diagnoses only possible with high suspicion and with immunohistochemistry confirmation.
