Microsurgical Clipping of Paraclinoid Aneurysm

Study Objective

To learn how to clip through fenestrated clip in paraclinoid aneurysm.

Design

  Through pterional approach and trans-sylvian dissection optico-carotid cistern was exposed and anterior clinoid was drilled intradurally, proximal dural ring was sharply cut and paraclinoid aneurysm was clipped by a fenestrated clip parallel to carotid.

Setting

Comfort Nursing Home, a private hospital in Dhaka, Bangladesh.

Subject/Audience

Neurosurgeons/Neurologists/Residents.

Intervention

Demonstration of pterional approach for clipping of paraclinoid aneurysm and its steps were intervened.

Results

Avoidance of complications like intraoperative rupture, carotid artery injury etc were achieved successfully.

Conclusion

  The steps of paraclinoid aneurysm clipping is well established. Prior anatomical concepts particularly drilling of anterior clinoid, proximal dural ring exposure and relationship of aneurysm to carotid artery is key to success.
