Abstract
Objective: A high protein weight loss diet program (HP) vs a high carbohydrate weight loss diet (HC) program were used to treat
obese subjects with newly diagnosed Type 2 Diabetes (T2DM) to determine the effects on remission of T2DM, cardiovascular risk
factors (CVRs), incretins (GLP-1, GIP), inflammation (IC), metabolic parameters, and muscle and fat body mass.
Research Design and Methods: Twenty-four obese women and men newly diagnosed with T2DM on no medications treatments
were recruited and randomized to either a high protein-low carbohydrate (HP) (12 subjects) or a high carbohydrate-low protein
(HC) (12 subjects) weight loss programs for 6 months in this randomized controlled clinical trial. Subjects were provided all foods for
6 months with daily meal menus which were dispensed weekly along with weight determinations. At baseline Oral Glucose Tolerance
Test (OGTT) and Meal Tolerance Test (MTT) and DXA were done with determinations of glucose, insulin, inflammatory markers (IC),
CVRs, incretin (GLP-1, GIP), and repeated after 6 months on the subjects on the respective diets.
Results: After 6 months on the HP diet program100% of the subjects had remission of their T2DM to Normal Glucose Tolerance
(NGT), whereas only 8% of subjects on the HC diet had remission of T2DM, although both subject groups had similar significant
weight losses. Insulin sensitivity, Cardiovascular risk factors, inflammatory cytokines and incretins (GLP-1, GIP) all exhibited
significant improvements along with increased per cent muscle body mass with the HP compared to the HC diet group which lost
muscle mass at 6 months.
Conclusion: Subjects on the HP weight loss diet program achieved 100% remission of T2DM to NGT at 6 months along with significant
improvements in cardiovascular risk factors, incretins, inflammation, metabolic parameters, and muscle retention compared to the
HC weight loss diet program. These improvements with the HP weight loss diet were achieved using foods available at local grocery
stores at a cost of only $15 per day.
