A 66-year-old woman with chronic atrial fibrillation, coronary artery disease, and left ventricular dysfunction, presented with gastrointestinal bleeding and respiratory distress. She was hemodynamically unstable, leading to immediate blood transfusions and treatment with pantoprazole infusion and antibiotics. She was on dabigatran 150 mg, since last 2 years and aspirin 75 mg, her 2 D echocardiography showed left ventricular dysfunction with LVEF 40-45%. An upper GI endoscopy revealed ulcerative exfoliative esophagitis, potentially linked to dabigatran use (Image 1-4).
